Structured Institutional Memory
Any discussion message can be promoted into a formal Decision (POST /api/v1/channels/:id/decisions) or an assigned Follow-up task with a due date and owner (POST /api/v1/channels/:id/followups).
Decisions cannot be silently deleted. When a decision changes, authorized leaders issue a superseding decision (POST /api/v1/decisions/:id/supersede) that marks the previous record as superseded while linking both in an auditable chain.
